A 19-month-old child died from serious injuries sustained while at the Illuminations Child Care facility in Wasilla. The incident occurred on Thursday, the child died on Sunday.

According to reports, the child, identified as Faith Phillips, was playing on playground equipment outside of the facility, when she stuck her head through a hole in the equipment while standing on a toy. She slipped from the toy and the strangulation resulted.

The unfortunate event occurred on the morning of September 6th. The police were called at about 11 am and when they arrived, the child was recieving CPR but not breathing.

The incident is under investigation by the Department of Health and Social Services and the facility has been closed for an undetermined amount of time.

The playground equipment as well as surveillance video was taken from the Childcare facility, but police say that the area where the incident occurred is viewable as it was in a blind spot.
